Registrar of Vital Statistics

The Registrar of Vital Statistics is charged with recording, maintaining and making available records of birth, marriage, death and domestic partnership occurring in Freehold Borough.  Copies of the following records may be obtained from the Registrar’s Office if the birth, death, marriage or domestic partnership occurred in Freehold Borough:

-           Birth Certificates
-           Marriage Certificates
-           Death Certificates
-           Domestic Partnership

Marriage License

If the bride or groom is a Freehold Borough resident, the couple applies for their Marriage License in Freehold Borough at the Registrar’s Office, 51 West Main Street, Monday – Friday, 8:30 AM –
4:00 PM – Please call to make an appointment.

The bride, groom and a witness 18 years or older, who knows the bride and groom, must appear together at the Registrar’s Office to apply for the marriage license.

The bride and groom must bring the following information:

-           Proof of Residence
-           Social Security Cards
-           Raised Seal Birth Certificate
-           $28.00 fee payable in cash, check or money order, made payable to, “Freehold Borough”.

If the bride or groom has been married before, a raised seal divorce decree or death certificate of previous spouse is required when applying for the new marriage license.

Obtaining Birth, Death of Marriage Record

In accordance with the Governor’s Executive Order #18, only the following persons are entitled to a certified copy of a vital record:  subject, subject’s parent, legal guardian or legal representative, spouse, child, grandchild or sibling, if of legal age.

In order to receive a certified copy of a birth, death or marriage record, you must complete the application below and send the following information in with your request:

-           A copy of a photo ID showing your address – OR
-           On a photo ID without address, you must submit one other form of ID showing address
-           Two alternate forms of ID showing address, i.e. vehicle registration, insurance card, voter             registration, passport, green card, county ID, school ID, utility bill.
-           Fee of $15.00 in cash, check or money order made payable to Freehold Borough.
